SUMMARY:

Introduction: Among the numerous sequelae caused post-stroke, cognitive deficits stand out, negatively impacting individuals. In view of this, the Mini-Mental State Examination (MMSE) is a widely used instrument for screening cognitive status in the post-stroke population. However, there are no psychometric data for the MMSE for this population from the version translated for Brazil. Objective: To evaluate the intra- and inter-examiner reliability of the MMSE for the post-stroke population in Brazil. Methods: Thirty post-stroke individuals were included, recruited non-probabilistically, for an in-person assessment with the application of the MMSE by a trained examiner (E1). For inter-examiner reliability, two other examiners (E2 and E3) watched the assessment content through a video recording, scoring the test independently. To investigate intra-examiner reliability, E1’s score in the in-person assessment was compared with a new assessment by the same examiner, but now through the videos, 7 days after the first analysis. Inter- and intra-examiner reliability was observed through the Intraclass Correlation Coefficient (ICC), models ICC2,1 and ICC3,1, respectively. Results: The results demonstrated excellent inter-examiner reliability in the total MMSE score and in the analysis of the subitems. Regarding intra-examiner reliability, the instrument showed excellent reliability. Conclusion: The Brazilian version of the MMSE showed excellent reliability, both inter- and intra-examiner, for the studied post-stroke population. However, application in post-stroke patients requires careful adaptations to ensure that the results truly reflect the individuals’ cognitive abilities.
