SUMMARY:

Introduction: The increase in life expectancy and in the older adults’ population observed worldwide, despite showing advances in quality of life and health conditions, presents a challenge for the health system due to the increase in the number of chronic conditions that accompany aging. The occurrence of falls and hip fractures are among these conditions that affect the health of the older person and increase the demand for health services. At the same time, data suggest that the occurrence of falls and fractures does not occur homogeneously between men and women, being more common in females, exposing the need for understanding this discrepancy and which factors are associated with falls and fractures according to the gender. Objective: The aim of this study is to evaluate the factors associated with the occurrence of falls and hip fractures among older men and women using data from the National Health Survey (PNS) carried out in 2013. Method: This is a cross-sectional study with data from the PNS 2013 with older people aged 60 and over who were also the resident selected for the interview. A total of 11,177 older people were included in the present study, with 4,555 men and 6,622 women. Two questions "fall in the last 12 months" and "hip fracture in the last 12 months" were used to classify the participants into the groups "non-fallers" (who did not report falls), "fallers without a fracture" (who had suffered falls but did not suffer a fracture) and “fallers with a fracture” (those who suffered a fall with a hip fracture). Socioeconomic data, mental health variables, presence of chronic diseases, functionality, health status and physical exercise were also evaluated. Data analyses were performed using the Chi-squared test, considering a p-value < 0.05. Results: Of the older people interviewed, 7.7% (n=1,825) reported a fall in the last 12 months. Of these, 67.6% were women and 32.4% were men. As for the occurrence of hip fractures resulting from the fall, of the 1,825 participants, 6.6% (120) reported a fracture. Of these, 70.9% were women and 29.1% men. There was a significant association between the presence of falls and fractures and marital status only for women (p<0.001), with a higher prevalence of not living with a spouse among those who reported falls and fractures than among the non-fallers. The presence of hearing and intellectual impairments was significantly associated with falls and fractures only among men, whereas the association with visual impairment was significant only among women. Regarding chronic diseases, a significant association was identified between falls and fractures with a higher prevalence of hypertension (p=0.021) and stroke (p<0.001) only in men, whereas only in women we found significant associations for diabetes (p =0.021) and high cholesterol (p=0.018). The relationships between reports of falls and fractures and other chronic diseases, exercise practice, mental health variables and functionality were significant for both men and women.
