SUMMARY:

INTRODUCTION: Osteoarthritis (OA) is a degenerative and progressive joint condition characterized by the formation of osteophytes and subchondral sclerosis, leading to pain and disability. For conservative treatment, physical exercise is described as the most effective intervention, as individuals with this condition often present with not only pain but also muscle weakness and balance deficits. Additionally, it has been observed that individuals with OA also show signs of central pain sensitization. Transcranial Direct Current Stimulation (tDCS) is a non-invasive neuromodulation technique that has been studied for its ability to induce changes in pain perception through cortical excitability.

OBJECTIVES: To evaluate the effects of the combined application of tDCS with a specific exercise protocol for knee osteoarthritis on pain and functional capacity.

METHODS: This is a double-blind randomized controlled trial involving individuals diagnosed with knee osteoarthritis. Inclusion criteria are a diagnosis of knee OA for at least three months, age 45 or older, pain > 3 on the NRS, and not undergoing physical therapy for at least 60 days. The study's primary outcomes will be pain and disability, assessed using the following instruments: NRS, WOMAC, Lequesne, KOOS, BP-CSI, Tampa Scale for Kinesiophobia, and FABQ. For the interventions, the experimental group will receive tDCS application for 20 minutes on the primary motor cortex area, followed by a 40-minute protocol of lower limb muscle strengthening exercises, twice a week for five weeks, totaling 10 sessions. The control group will receive SHAM tDCS, followed by the same exercise protocol. Statistical analysis will be conducted using a mixed repeated measures ANOVA, with time-group interaction analysis. A 95% confidence interval and a 5% significance level will be considered. This research was approved by the Research Ethics Committee (CEP) of the Faculty of Health Sciences of Trairi (FACISA/UFRN) under the approval number: 6.801.827.

EXPECTED RESULTS: It is expected that the findings of this study will contribute to the understanding of a management approach for chronic pain in individuals with knee osteoarthritis, as well as expand the scope of knowledge regarding the combination of physical exercise with neuromodulation. Given that transcranial direct current stimulation is emerging as an adjunctive technique in the treatment of chronic disorders, it has gained increased attention in recent years.
